Withdraw from contract here
If you concluded a contract with Atlaspraxis Seeham via this website, you can withdraw from it here within the withdrawal period – without giving any reason.
The period is fourteen days from the day the contract was concluded. Appointments that were merely arranged with me by telephone or email are not covered by the statutory right of withdrawal – simply cancel such appointments, free of charge up to 24 hours in advance (Terms, section 5).
Fill in the form and send it with “Confirm withdrawal”. You will immediately receive a confirmation of receipt stating the content, date and time at the email address you provide. No reason is required. Everything else about the right of withdrawal is in section 8 of the Terms.
Please enter your name, the email address for the confirmation of receipt and details that let us identify the contract – for example the session date or “block of ten, bought on …”. No reason is required.
